Louisiana Residents: Applicants for Licensure by Examination/Endorsement and Louisiana LPN requirements

Louisiana resident applying for licensure by examination/endorsement and Louisiana LPNs who are required to obtain criminal background checks as directed by the Louisiana State Board of Practical Nurse Examiners (LSBPNE) should follow the instructions below:

Please Note:  Louisiana and FBI criminal background checks are processed by Louisiana State Police via IdentoGo.  LSBPNE receives the results of processed background checks.

Instructions to apply for Louisiana/FBI criminal background check:

1. Visit https://uenroll.identogo.com/ to begin the scheduling process.
2. Select, “Schedule or manage appointment”
3. Enter the six-digit service code for LSBPNE - 27N3R6
4. Enter the requested information. **It is important that the information entered matches the form of ID that will be presented at the time of the fingerprint appointment.
5. There is no additional code required for LSBPNE.
6. Continue through the additional screens and enter the requested information.
7. Enter your preferred zip code to show the five nearest sites for fingerprinting.
8. Choose your fingerprint site and schedule your appointment.
9. The confirmation page will summarize the details of the appointment and provide an estimated payment amount. Payment will be due at the time and place of your appointment. Forms of acceptable payment are: business check, money order, or credit card (card holder must be present at the time of payment)
10. An email confirmation will be sent.
