you have ever held a Louisiana LPN license, you do not qualify for
endorsement.
You
must instead request to reinstate your Louisiana license. |

IMPORTANT
!!! FORM ONE IS TWO PAGES (SIDE A AND SIDE B). BOTH
PAGES MUST BE SUBMITTED OR MAY RESULT IN A DELAY IN ISSUING A TEMPORARY
PERMIT AND/OR LICENSE.
