Registration:

***   TO RENEW ONLINE, YOU MUST BE A REGISTERED USER.   ***

To register as a user, you will need the following information:

  • Your LPN license number

  • The last 4 digits of your social security number

  • Date of birth

  • A valid email address (this will become your username and this is where your forgotten password will be sent)

  • A password

NOTE:  Once you have a registered user account, you can use your account to report an address change at any time during the year.  You can do this under the "My Account" menu item after you have logged in.


To renew online:

If you are already registered as a user and want to renew online you must first login using the following information:

  • The username you provided when you registered (your username is the email address you gave)

  • The password you created when you registered (if you do not remember your password, you can click the "Forgot Password" link when you go to the LOGIN page)

Once you have successfully logged into the online renewal system you can begin the renewal process.

 

You will need the following items and information in order to renew your license online:

  • A credit card (Visa or Mastercard only)

  • The name, address and phone number of your current place of employment as an LPN.  If you are not currently employed as an LPN you should have the name, address and phone number of your last place of employment as an LPN.  If you are not currently employed as an LPN but have worked as a volunteer or have provided care to a family member, you should have information related to where you worked and/or for whom you provided care.

NOTE:  If you have a name change to request, you must renew your license through the mail.  There is no charge for a name change made during license renewal.  Just submit your request with your renewal application accompanied by a copy of a marriage license, divorce decree or other legal document which supports your name change request.  

REMEMBER:  The name on your license is the name you must use when signing nursing notes, medication administration records, etc.

NOTE:  TO PROTECT YOUR PERSONAL INFORMATION, YOU MUST CLOSE YOUR INTERNET BROWSER WHEN YOU HAVE COMPLETED YOUR ONLINE RENEWAL.  YOU SHOULD NOT ALLOW SOMEONE ELSE TO RENEW RIGHT AFTER YOU WHILE THE BROWSER IS STILL OPEN.


Frequently asked questions:
Q.   I tried to renew online and got a message that I was not eligible for online renewal.  Why can't I renew online?

A.   Some of the possible reasons for receiving a message indicating you are not eligible to renew online include:

  • Delinquent or inactive license status

If you do not hold a current license, you are not eligible for online renewal.  You must request renewal information from the office and renew by mail.  Call the office and request a renewal application or email your renewal request to renewals@lsbpne.com

 

  • Disciplinary action

If your license is currently on probation, you are not eligible for online renewal.  You must complete and submit the renewal application you received in the mail.  You may not renew your probationary license as a "walk in".  Your application will be processed and your renewal probationary license will be mailed to the address you provide.

 

  • Student loan default:  

If you have a student loan that is in default with the Office of Student Financial Assistance (OSFA), you are not eligible for online renewal at this time.   You must make satisfactory payment arrangements with OSFA.   Once OSFA notifies our office that you have done so and that you are eligible to renew, we will send you notification by mail.   You can contact OSFA at 1-800-256-6882.

 

 
Q.   I don't believe any of the above apply.  Is there any other reason I might be getting a message indicating I am not eligible for online renewal?
A.    Yes.  There may be errors in the information you entered when creating your user account.   To check your information: 
  • Log into your online account.
  • On the initial screen, look for the "MY ACCOUNT" link in the menu bar and click on it.
  • On the next screen, you should see an "UPDATE ACCOUNT INFORMATION" link.  Click on it.
  • Check all of your information.  If any of the information is not correct, fix the errors and then select the "UPDATE" button.
  • After you receive the update successful message, click the "RENEWALS" link in the menu bar.
NOTE:  If you do not find any errors in your account information, please email renewals@lsbpne.com and include the following information:
  • Name

  • LPN license number

  • Date of birth

  • Last 4 digits of your social security number

  • Return contact information

We will then check your information and send you a reply.

NOTE:  Allow two weeks for your license to be processed and mailed to you whether you renew by mail or by use of the online renewal system.  Please do not contact the office to check on the status of your license before two weeks have elapsed.  If you have not received your license in the mail after two weeks have elapsed, notify the office by phone or email.

REMEMBER:   LPN licenses expire January 31st of every year.
