  To Report a Change in Name or Mailing Address
Name Change:
It is the responsibility of every LPN with a Louisiana license to notify the Board office immediately in the event of a name change.  Notification must be in writing and a copy of a legal document granting a name change, as well as a copy of your current LPN license, must be included.  The fee is $30, payable by cash (in person) or money order (to the LA STATE BOARD OF PRACTICAL NURSE EXAMINERS).  Mail to 3421 North Causeway Blvd, Ste 505, Metairie, La. 70002.

Address Change:

Address changes may be submitted over the phone (504-838-5791) or in writing (include LPN license number and social security number).  

 

NOTE:

RETURNED MAIL WITH THE POST OFFICE FORWARDING STICKER DOES NOT CONSTITUTE NOTIFICATION BY THE LICENSEE.  

IT IS THE LICENSEE'S RESPONSIBILITY TO INFORM THE BOARD OF AN ADDRESS CHANGE.

Address changes made on license renewal notices must be made in the space provided on the face of the notice. 

Written communications, applications, and licenses issued by the Louisiana State Board of Practical Nurse Examiners (LSBPNE) are mailed to the LPNs who hold a current license to practice in the state of Louisiana.  All documents are mailed to the most recent address on file with the Board.  By law, LPNs must immediately notify the Board of an address change.
