| If your Louisiana LPN license has been lost, damaged, destroyed or stolen, you must contact our office to request a "Lost License Form." Call 504-838-5791. | ||
NOTE: The following
documents are required to accompany the form:
If you do NOT have a driver's license, then we will need a copy of 2 forms of personal identification (such as Social Security card, paycheck stub showing your name and address, a work badge with your name and picture on it (should you have one), etc. |

| The form must be completed and submitted to our office with a $30.00 money order (no personal checks), payable to "Louisiana State Board of Practical Nurse Examiners." Forms not accompanied by the required fee will be returned. | ||
