If your
Louisiana LPN license has been lost, damaged, destroyed or stolen, you
must contact our office to request a "Lost License Form."

The form must be completed and
submitted to our office with a $30.00 money order (no
personal checks), payable to "Louisiana State Board of Practical
Nurse Examiners." Forms not accompanied by the required fee
will be returned. |

|
|
|
NOTE: We do not issue a
replacement or "new" license. Instead, we issue a
verification card, with our seal on it, providing your LPN license
information and verifying you have a current license. If any employer has questions concerning the
verification card, please refer them to our office. |
