Before
using our online system to verify licensure status, please read the
following: |

|
|
Verification
of LPN Licensure |
|
|
|
|
- Verification obtained through our
online system does not ensure that the person presenting him/her self
is, in fact, the rightful owner of the license. You should
always visually inspect both the LPN license and (at least on
initial hire) the LPN's drivers license (or some other State issued
photo I.D.) to compare signatures and photos against the live
signature and physical likeness of the LPN presenting him/her self to
you. You should have the LPN's license number before entering
the verification site as it may be needed to narrow down the search
results and to ensure you have the correct LPN.

- LPNs are required to have their
license with them at work. Employers should regularly spot check
licenses by requesting licensure presentation "on demand".
|
|
- Check to be sure that the license
does not appear to have been altered in any way.
|
|
- Check the face of the license to
determine whether or not it is stamped with "PROBATION" or
with any other marking indicating that the license is encumbered (and
may require special handling by an employer).
|
|
- Despite our best efforts to inform
employers of disciplinary actions taken by the Board, changes in licensure
status as a result of disciplinary action can happen at any time,
including after an online verification has been obtained.
Reminder: Our office will verify disciplinary status free of
charge, during normal business hours, by phone, mail, fax or email.

Verification
of Temporary Work Permits Issued to Exam and Endorsement Applicants |
|
|
|
- Temporary work permits are not
included in the online verification system at this time. To
verify a permit, you must submit a written request, on letterhead,
providing the applicant's name and permit number, along with a $5.00
fee per applicant, payable by company check or money order, to:
Louisiana State Board of Practical Nurse Examiners (or LSBPNE), 3421
North Causeway Blvd, Ste 505, Metairie, LA 70002.

- The fee is $5.00 per verification.
Fees are *** NON REFUNDABLE *** Payment is
accepted by Visa, MasterCard, or Discover.
|
|
- Ideally, the data for our online
verification system will be uploaded after the close of each business
day. HOWEVER, before ordering any license verification(s),
you should always check the bottom (right corner) of this page for the
DATE and TIME the data has been updated.
|
|
