Baker, Denise pending disciplinary decision guilty of a crime; performing procedures beyond the scope of practical nursing; terminated from St. Tammany Hospital for allegedly obtaining a prescription with an altered prescription form
Bercegeay, Dwain eligible/probation 3 years with stipulation arrest on February 3, 1995 possession of cocaine, January 6, 1988 attempted unlawful acquisition of CDS-Vicodin by forgery
Blade, Scott suspended indefinitely is habitually intemperate or is addicted to the use of habit forming drugs; arrested for Driving While Intoxicated
Cummings, Sonya remains summarily suspended unfit or incompetent by reason of negligence, habit or other causes; habitually abused cocaine and alcohol
Dalon, Sue suspended 2 years/stayed/2 years probation with stipulations is habitually intemperate or is addicted to the use of habit forming drugs; did exhibit signs of being chemically impaired while on duty at Chateau Living Center as an LPN and had to seek treatment for a possible drug overdose. Terminated from Chateau Living Center for possessing unauthorized prescription medication of a patient which was found in her purse.  That she has sought treatment for chemical dependency.
Dixon, Brenda consent agreement/suspended 12 months is guilty of a crime; is guilty of unprofessional conduct; failure to practice practical nursing in accordance to the standards expected; improper use of drugs, medical supplies or patients records; committing any act that adversely affects the physical or psychosocial welfare of the patients; arrested in Calcasieu Parish for 1 count of attempting to obtain CDS by fraud (hydrocodone); terminated from Lake Charles Care Center for being in possession of two (2) methadone tablets specifically ordered for patients in her care
Ducote, Carol remains summarily suspended is guilty of a crime; convicted of Theft $100 - $500, Issuing Worthless Checks > $500, Possession of CDS IV
Dupree, Hollace suspended is guilty of a crime; is habitually intemperate or is addicted to the use of habit forming drugs; arrested for felony issuing worthless checks and attempting to obtain CDS by fraud in January, 1998; arrested April, 1998 for access device fraud; currently receiving treatment for opiate dependence
Evans, Jeanne suspended 2 years/stayed/2 years probation with stipulations is habitually intemperate or is addicted to the use of habit forming drugs; is guilty of unprofessional conduct; failure to practice practical nursing in accordance to the standards normally expected; on or about November 2, 1998 was terminated from Willis-Knighton South Hospital for failing to document controlled medications and removing from the Pyxis Medication system controlled medications for patients who were discharged and had no order for the drug
Finch, Kimberly reinstated/2 years, probation with stipulations is habitually intemperate or is addicted to the use of habit forming drugs; is guilty of unprofessional conduct; failure to practice practical nursing in accordance to the standards expected; improper use of drugs, medical supplies or patients' records; on or about December 14, 1998 terminated from Slidell Memorial Hospital for being chemically impaired on duty; falsified patient record, tested positive on a urine drug screen of which she did not have a prescription and was found to be in possession of hospital drugs on her person
Grayson, Greta suspended/stayed/2 years probation w/stipulations is guilty of unprofessional conduct; failure to practice practical nursing in accordance to the standards normally expected; failed to administer medications to a patient in her care and did administer a tube feeding inappropriately while employed as an LPN on or about November 9, 1998 at Ochsner Hospital; worked 8 days as an LPN without a license to do so.
Guidry, Linda suspended indefinitely is habitually intemperate or is addicted to the use of habit forming drugs; admittedly addicted to the use of habit-forming drugs namely "crack" cocaine, speed, alcohol, marijuana and is currently receiving treatment for substance abuse
Hedrick, Debbi reinstated/3 years probation with stipulations guilty of a crime; has not practiced as a practical nurse in 4 or more years; arrests for Theft, Simple Battery, Disturbing the Peace, Theft of Goods and Theft < $100 between 1994 and 1996
Huff, Virginia reinstated/2 years probation stipulations is habitually intemperate or is addicted to the use of habit forming drugs; tested positive for marijuana on October 29, 1999
Manchester, Ourelia reinstated/letter of reprimand/$250 fine is guilty of fraud or deceit in procuring or attempting to procure a license to practice practical nursing; submitted a falsified 1999 renewal application and practiced practical nursing without a current license
May, Antoinette suspended is guilty of a crime; falsified her 1997 renewal application by answering no to the question in the past 5 years have you ever been arrested.  Arrested on or about August 10, 1996 for shoplifting and on or about April 14, 1998 for possession of stolen things.  Failure to provide the Board with requested information.
McKenzie, Patricia suspended 2 years/stayed 2 years probation with stipulations is unfit, incompetent by reason or negligence, habit, or other causes; is guilty of unprofessional conduct; failure to practice practical nursing in accordance with the standards normally expected; falsifying records; on or about October 6, 1998 terminated from Riverview Medical Center for narcotic administration discrepancies and a positive urine drug screen for Demerol
Melohn, Heather suspended is habitually intemperate or is addicted to the use of habit forming drugs; is guilty of unprofessional conduct; performing procedures beyond the authorized scope of practical nursing; improper use of drugs, medical supplies, or patient's records; misappropriating personal items of an individual or the agency; unauthorized self-administration of at least 10 doses of Nubain from the supply of the LA Practice Clinic
Miller, Deirdre eligible/probation 2 years with stipulations convicted on June 17, 1987 for possession of marijuana; has completed a chemical dependency treatment program
Miller, Jeanette consent agreement/suspended indefinitely is habitually intemperate or is addicted to the use of habit-forming drugs; on or about September 24, 1998 terminated from West Jefferson Medical Center for theft of hospital property, falsification of patient's records and narcotic diversion
Mitchell, Mischell remains summarily suspended guilty of unprofessional conduct; falsifying records; arrested for Attempting to Obtain CDS by Fraud
Ramos, Suzanne suspended 6 months/medical ethic review course guilty of unprofessional conduct; performing procedures beyond the authorized scope of practical nursing; misappropriating personal items of an individual or agency; addiction to the use of Vicodin
Rollins, Joseph 1 year probation with stipulations guilty of a crime; guilty of Theft of Property > $500.00
Schexnyder, Wallace suspended is unfit, or incompetent by reason of negligence habit or other causes; tested positive for benzodiazepine, cocaine on a urine drug screen done for cause while employed as an LPN at Southdown Care Center
Smith, Carolyn consent agreement/1 year probation with stipulations is guilty of unprofessional conduct; tested positive on a urine drug screen for marijuana
Stewart, Suszanne suspended is guilty of unprofessional conduct; failure to practice practical nursing in accordance to the standards expected; performing procedures beyond the authorized scope; administration of APAP 10/650 to a resident in her care when in fact the medication was not ordered by the physician or accounted for based on requirements for the handling of controlled substances
Varnado, Brenda refresher course/reinstated on 2 year probation with stipulations is unfit or incompetent by reason of negligence, habit or other causes; is habitually intemperate or is addicted to the use of habit-forming drugs; is guilty of unprofessional conduct; misappropriating personal items of an individual or agency; falsifying records; in January 1994 diverted Demerol for self-use while employed at Chalmette Medical Center and in April, 1994 gross narcotic discrepancies (namely Demerol) while employed at Mercy/Baptist Hospital
